Accidental Death and Dismemberment Insurance

In the United States, accidents are the number one cause of death among people under the age of 44, and the fifth leading cause of death overall according to the National Vital Statistics Report published by the Centers for Disease Control and Prevention in 2010. Accidental Death & Dismemberment (AD&D) insurance through Provident and underwritten by Life Insurance Company of North America or Cigna Life Insurance Company of New York can help provide added protection for volunteer firefighters and emergency responders.

  • Our AD&D coverage includes a Seat Belt Benefit which is an additional 10% of the AD&D benefit up to a maximum of $10,000 that may be payable if an insured person dies as the result of a covered accident in any vehicle while he or she was wearing a seat belt.
  • Eligible volunteer firefighters and emergency responders are covered by our AD&D insurance while at work and when they are off-duty. Members are covered 24 hours a day anywhere in the world, and this includes riding as a passenger in an aircraft being used for transporting passengers.

In order to provide a quote for our Accidental Death & Dismemberment insurance, we need to know some basic information including how many members the volunteer fire department or emergency service organization would like to cover, as well as the benefit limit (up to a maximum of $500,000).

LIMITATIONS

Any covered loss must occur within 365 days of the covered accident. Benefits for loss of life will be reduced by any paid or payable accidental dismemberment benefits. Benefits (principal sum) may reduce depending on on the final plan design. The actual benefits paid may vary by class, plan design, and the extent of injuries sustained as a result of a covered accident. The acceptance of a renewal premium is optional with the policy holder.  We may terminate coverage on or after the first anniversary of the policy effective date. The policy has additional terms under which it may be continued in force or discontinued. Seat belt benefit is paid for a covered accident resulting in loss of life while wearing a seatbelt and excludes a mobile home or any motor vehicle which is used in mass or public transit. Hepatitis C benefit is paid if the covered person during the performance of occupational duties acquires and tests positive for Hepatitis C within one year and as a result of a covered accident (policy conditions apply). Firefighter & ES Personnel Line of Duty Benefit is payable for a covered accident resulting in loss of life while in the line of duty and excludes injury resulting from maintenance, repair or cleaning of firearms and injury sustained in consequence of the illegal use of firearms by the covered person. The benefits described do not cover injuries or loss of life as a result of: intentionally self-inflicted injury, suicide or any attempt thereat, while sane or insane; commission or attempt to commit a felony or an assault; commission of or active participation in a riot, insurrection or terrorist act; bungee jumping; parachuting; skydiving; parasailing; hang-gliding; declared or undeclared war or act of war; flight in, boarding or alighting from an Aircraft or any craft designed to fly above the earth’s surface, except as: a fare-paying passenger on a regularly scheduled commercial or charter airline, a passenger in a non-scheduled, private Aircraft used for pleasure purposes with no commercial intent during the flight; or a passenger in a military aircraft flown by the air mobility command or its foreign equivalent; participation in any motorized race or contest of speed; an accident if the covered person is the operator of a motor vehicle and does not possess a valid motor vehicle operator’s license, except while participating in a driver’s education program; sickness, disease, bodily or mental infirmity, bacterial or viral infection or medical or surgical treatment thereof, except for any bacterial infection resulting from an accidental external cut or wound or accidental ingestion of contaminated food; medical or surgical treatment, diagnostic procedure, administration of anesthesia, or medical mishap or negligence, including malpractice; voluntary ingestion of any narcotic, drug, poison, gas or fumes, unless prescribed or taken under the direction of a physician and taken in accordance with the prescribed dosage; a covered accident that occurs while on active duty service in the military, navy or air force of any country or international organization (Reserve or National Guard training are not excluded until training extends beyond 31 days).
